A Patient has the right to:
- Receive all dentally necessary care that is available from our office or receive a referral to another provider.
- Considerate and courteous care with respect for personal privacy and human dignity.
- Information concerning their diagnosis, treatment, and prognosis (i.e. clear concise communication from our staff, access to their medical records in compliance with the law.)
- Involvement in the decision making concerning their treatment (i.e. signing informed consent prior to treatment, right to refuse to the extent permitted by law.)
- Confidentiality of information concerning one's treatment.
- File a grievance and to be given due process.
- Obtain a Notice of Privacy Practices.
A Patient has the responsibility to:
- Comply with the prescribed dental instructions from the office.
- Provide all accurate past and present medical, dental and personal information to our office.
- Keep appointments for care and give required notice to our office when canceling appointments.
- Maintain one's health and participate in the decisions concerning treatment.
- Read all material issued from the office.
- Ask questions of the provider if they don't understand.
- Promptly meet financial obligations.
